What to Expect at Your First Visit
Your first appointment is all about getting to know you and your smile. During your visit, we will:
Review your medical and dental history
Take any necessary X-rays
Perform a gentle, comprehensive exam
Discuss any concerns or goals you have
Create a personalized care plan moving forward
We take time to listen, answer questions, and make sure you feel fully informed before any treatment.
New Patient Forms
At your first appointment you will need to complete the following forms. You can save time and print them out now, email them to us or bring them with you.

For all patients. This includes contact info, insurance info and emergency contact info.

Medical History Form
For all patients 18 and over. Includes questions related to your current and past health.

Child Medical History Form
For any patient under 18 years of age. Includes questions related to current and past health.

General Consent Form
To allow us to treat you in alignment with the agreed upon plan.

HIPAA Consent Form
Consent for use of health info to carry out treatments.
